I am willing to go through the Super Parent Support Program as listed here and to be responsive to the communications I receive from the Brady Wynn Foundation.

If at any time I appear to be unsafe, I consent to Brady Wynn Foundation exercising its right to request a Wellness Check to be conducted by the local authorities.

We at Brady Wynn Foundation reserve the right to adjust service commitments for any reason. We aim to serve families immediately following the diagnosis/complications, so services are designed for this time period. Brady Wynn Foundation can stop services at any time due to lack of communication, commitment or respect.

If cleaning services are provided, I will remain on property while the cleaning team cleans my home and periodically check to ensure they are performing the work as determined prior. I will not hold Brady Wynn Foundation responsible for any missing or damaged items. Brady Wynn Foundation is not responsible for any issues that may arise as a family is choosing to participate or use the provided cleaning services.  If a parent is not comfortable, the service will be cancelled and will not be available for future use. All cleanings must be completed with in 12 months. If you miss a month, it will not be rescheduled. After the initial clean is complete let us know if you are not satisfied with your service and we will cancel remaining cleans. If you need to cancel due to hospitalizations or illness, please let Mandy know by email with in 24-48 hours. If it is shorter notice please let your cleaning location know as well as Mandy. If you no-show services will be suspended.

*If cleaning services are offered, we have a budget we must stick to, we are offering a monthly maintenance clean, not a deep clean or haul out.

If massage is provided, massage therapy will be provided by the use of a gift card. This is determined at the sole discretion of Brady Wynn Foundation.

*Please note, massage pricing varies by location. The massage value will not exceed $150 total.

Any appointments through the Program cannot be rescheduled, cancelled or moved. Any rescheduling, cancellation, or no-call/no-show may result in your services being cancelled. Exceptions may be made for hospitalizations, illness, accidents, etc.

Upon signing this form, any provided cleaning services must be used with in 12 months. Any unused cleaning program services remaining after the designated time period will be forfeited and no further services will be provided by The Brady Wynn Foundation.

If exercise program is selected, you agree for BWF to share your email and phone number with Kate from Love Yourself Strong and to communicate with Kate on a regular basis. Kate will reach to schedule the initial phone consultation. This consultation must be completed with 30 days of joining our program. Kate will communicate to you weekly for 6 weeks. If you do not respond back to her within the week, she will assume you no longer want this service.

6 week maximum personal training based off of your workout experience and wants. Main focus is to release endorphins and love yourself.

General Release, Waiver of Liability, and Statement of Informed Consent: I hereby agree that in consideration of Brady Wynn Foundation allowing me to participate in their Program, I do for myself and my heirs, executors, administrators, and assigns hereby release, waive, and forever discharge Brady Wynn Foundation, its Board of Directors and its officers, agents and employees, from any and all liability, claims, demands, actions, or causes of actions arising out of or related to my participation in The Super Parent Support Program. In no event shall Brady Wynn Foundation be liable for any incidental, consequential, indirect, statutory, special, exemplary or punitive damages, including, but not limited to, lost profits, loss of use, loss of time, inconvenience, loss business opportunities, damage to goodwill or reputation, or other economic loss, regardless of whether such liability is based on breach of contract, tort, strict liability or otherwise, and even if advised of the possibility of such damages or such damages could have been reasonably foreseen.

I shall indemnify, defend, and hold harmless Brady Wynn Foundation, its Board of Directors and its officers, agents and employees from any loss, liability, damage, or expense including reasonable attorneys’ fees and costs from any claim that may arise from or in connection with my participation in The Super Parent Support Program. If I fail to promptly indemnify and defend a covered claim, Brady Wynn Foundation shall have the right to defend itself, and in such case, I will promptly reimburse Brady Wynn Foundation for all of its associated costs and expenses.

I represent that I am at least eighteen (18) years of age, the legal guardian of the infant, and am fully competent to sign this agreement. This agreement shall (i) be binding upon me and my agents, representatives, assigns, heirs, and successors in interest; (ii) be governed by and construed in accordance with the laws of the State of Wisconsin without giving effect to any choice or conflict of law provision or rule; and (iii) any and all claims arising out of or relating to this agreement shall be brought in a state or federal court of competent jurisdiction in Ozaukee Co, WI. I consent to the personal jurisdiction of the state and federal courts located in Ozaukee Co WI. I waive (a) any objection to jurisdiction or venue, or (b) any defense claiming lack of jurisdiction or improper venue, in any action brought in such courts.
