Brady Wynn Foundation “Super Parent Support Program” Application
This form is required to be submitted before receiving services through the Brady Wynn Foundation. Our foundation aims to help families who have a child 2 years old or younger that receives a life-threatening neurological disorder diagnosis or suffers an accident causing life-threatening neurological complications. We encourage you to apply within 3 months of the accident or diagnosis so we can be there to provide support during the initial adjustment into this new way of life. Your child must be living to apply.
Not only do we provide this assistance, but we are also focused on building a supportive, safe community for families to connect with one another. It is our goal that every family has someone who will listen and understand, without judgment, and share in your hope and joy when things are going well. We want to set families up for success in the long run, therefore, we ask all families who go through our program to join our online support, so it is always there if you need it.
This desire to join our community for the long haul is a key component of being accepted into our program. Applicant must be the legal guardian and primary caregiver for the child. Applicant must live in the United States. A member from our foundation will email you once we receive this form to let you know if we are going to proceed with your application. Due to the volume of applicants and the overwhelming need, sadly, we are not able to help everyone that applies. Thank you for taking the time to fill out our form.
